Providing Multi-Disciplinary Follow-up for Survivors of Pediatric Malignancies

Mission Statement:

To optimize quality of life for children and young adult survivors of childhood cancer by providing focused risk-assessment information and screening for multiple physical and psycho-social impacts of disease and treatment.

Description:

Multi-disciplinary clinic with clinic staff including: pediatric oncologist, YES Clinic nurse (Velma Vigil RN); neuro-psychologists (Dr. Robert Annett and Dr. Richard Campbell); social worker (Yolanda Vinajeras MSW), clinic coordinator (Brenda Beauchamp Lobato), patient educator/child life specialist (Paula Dorris-Osborn)

Eligibility:

Patients must be at least 2 years off therapy and cancer-free ages 2-40+ years.

Treatment Summary:

Individual treatment summary developed for each patient prior to the clinic visit which includes: diagnoses, all chemotherapy agents received, radiation therapy, surgical procedures, transplants, significant complications, and adverse drug reactions/allergies.

Young Adult Transition Program:

Monthly YES clinic for young adults ages ~2-40+, Dr. Stephanie Gehres (Med/Peds) will evaluate these patients as well as rest of the clinic staff
