Comprehensive Community Support Services

CCSS is a recovery based model that forms a partnership with the client to assist them in reaching their individual goals. It is client centered in that we work w/ the individual and their families to help them live as independently and as fully as possible. It is based on the belief that people can and do recovery from mental illness.

• Help with applying for entitlements, general assistance, food stamps, and housing.

Referrals

Referrals are required and can be obtained from the individual’s doctor, nurse or social worker.

What can they do for me?
Help with applying for disability (SSI, SSDI), general assistance, food stamps, and housing. CSW’s also help the client w/ getting a PCP, dental work, and provide support through some legal issues.

How long will they help me?
The time period for CCSS is entirely up to the needs of the client; one month to years.
